Press CTRL+H to turn the EXTRA option on, this will help you to center the circle to cut out the hole. After centering your circle as show below press Delete to delete the selection.

Step2: At this point all you should have is two layers the Background transparent layer and the layer with the CD with the hole in the center. Now we need to create the ring around the center of the CD. Create a new layer (SHIFT+CTRL+N) or click on Layer New Layer. Click on Custom Shapes and select the shape shown below.

Draw the shape around the center hold of the cd to get an effect shown in the image below, change the color of the shape to #cecece.

Step3: Hold CTRL and Click on the layer with the CD to select it. It should look like the image below.

Now, create a new layer (SHIFT+CTRL+N) click on the Polygonal Lasso Tool  make sure Intersect with selection is selected . We need to create the reflection coming off the CD. With the Polygonal Lasso Tool draw a triangular shape lasso as shown in the image below.

Now click on the Gradient tool  its usually hiding behind the paint bucket. Click here to download the gradient below.

Using the gradient you just downloaded, create a gradient going from top to the bottom as shown in the image below.

Step4: Now repeat step 3 to add more reflections, until your cd looking like my image below.

Step5: Now hold CTRL and click to select all the layers except the background and press CTRL+E to merge them.

Hold CTRL and Click to select Press CTRL+E to merge layers

Now click on the Chop Tool, right click the CD image, and click Free Transform as shown in the image below.

Now enter the setting shown below in the task bar option that comes up after clicking on Free Transform.

Step6: We need to create the shadow. Create a new layer, Click on the Elliptical Lasso Tool and draw an ellipse shown in the image below.

Fill the selection with #bcbcbc, make sure the shadow layer is under the cd rom layer.

That is it, hope it came out to what you expected.
