I like the clean stile templates not with too much graphics or clutter.

(1) Open Adobe PhotoShop File> New

(2) Click View> Actual Pixel (Alt+Ctrl+0) now you will see the full screen view. Change the background color to #928f8a color

(3) Create a new layer (Layer > New>layer) than click ok

(4) Click on the Rectangular Marquee Tool and enter the setting below. Make sure you center the Rectangle in the middle of the page and fill it will white #FFFFFF.

(5) Right Click on the new Layer you just created and Blending Options.

     Double Click on Outer Glow and enter the setting below

     Now click on Stroke and enter the setting below. When your finish click ok.

(6) Now create a new layer and Click on the Rectangular Marquee Tool and enter the setting below.

  Place the rectangle about 3 inches from the top. Click here to see an example.

  After that step fill the rectangle with this color #799325.

(7) That's it pretty much now you can add your text and a banner. Here is my finished example
